Member (Grading) Card Replacement

If you need a Replacement Grading Card please use this form  If you have trouble downloading the form, an email with Rider Name, Club and where it is to be posted will suffice - membership@ponyclubvic.org.au   If you need it quickly please phone the office (during office hours 9-5 Monday - Friday) on 8685 8925 to order one. We need the full name of the rider and where the card should be sent (bear in mind that it needs to be signed by 2 club officials). There is a $6 charge for Replacement cards.
